The Case Manager collaborates with the SISU team to execute a vigorous case management strategy with proven-risk young people ages 14 to 24 years old engaged with services at the SISU Center. The position provides a great opportunity for a person looking to make a difference in the community. By helping to enrich the lives of SISU participants, this individual will contribute to reducing crime, increasing employment opportunities and fostering positive social-emotional development within the population we serve.
Responsibilities:
- Coordinate referral and intake process for new clients
- Conduct needs assessments and create/implement ISP’s (Individual Service Plans)
- Builds strong trusting relationships with proven-risk client population
- Manage and maintain a minimum caseload of (15-20) selected proven-risk youth.
- Meet regularly with clients, monitor progress, and make recommendations for services and programming
- Provide daily social/emotional support through regular face-to-face contact, and regularly evaluate clients’ progress and/or setbacks
- Facilitate programming including workforce development, life skills, social-emotional growth, etc.
- Maintain client records within the organization’s data management systems
- Build partnerships and coordinate services with community organizations
- Identify, establish and monitor post-secondary education, vocational training and job placement opportunities for youth
- Provides emergency assistance, crisis intervention, shelter assistance and support with court recommendations, conditions and mandates
- Attend staff meetings, professional development and program activities
- All other duties assigned
Qualifications:
- Bachelor’s in Human Services, Criminal Justice, Psychology, or related field
- Understanding of urban culture and proven-risk youth preferred
- Strong comfort level in approaching youth at different levels of social-emotional growth
- Ability to work sensitively and effectively in a multicultural setting
- Strong leadership, organization, and facilitation skills
- Comfort working within a team-oriented, professional environment
- Valid driver’s license and has own transportation
- Bilingual (Spanish/English) preferred
- Available to work a flexible schedule including days, evenings, weekends if necessary

Hours: Flexible work schedule which may include evenings and weekends
