Job Objective/ Summary: The Case Manager/Utilization Review Director is responsible for case management, discharge planning, and utilization review for Lawrence Memorial Hospital. Responsible for coordinating and providing care that is safe, timely, effective, efficient, equitable and patient centered. Handling case assignments, review patient progress and determining patient discharge. Helping patients achieve wellness and autonomy through multi-disciplinary therapy, nursing needs and patient/family interaction. The Case Manager/Utilization Review Director will enhance the quality of patient management, maximize satisfaction and promote cost effectiveness. The Case Manager/Utilization Review Director will be accountable for the full patient care cycle that includes, assessing, planning, implementing, monitoring, and evaluating actions required to meet the patient’s health and human service needs.
Job Education, Licenses/Certifications, Knowledge & Experience: Must hold a Bachelor of Science in Nursing (BSN), Must hold a valid and active license in good standing as an RN in the state of Arkansas, Completion of an accredited nursing program, Must be ACLS, PALS and CPR Certified, Strong knowledge of nursing principals, practices and procedures, Compassionate, patient-centered approach.
