The primary function of the Patient Support role is to help patients communicate with their healthcare provider to get the information needed to make decisions about their health care, and to provide interpretation when needed. After interactions with patients, the Patient Support will bring forward to the clinical team issues that may impact health outcomes. This role may also help patients schedule appointments with specialists, labs or for medical tests, and provide information regarding transportation options if needed. Additionally, Patient Support may screen patients for Social Determinants of Health (SDOH).
