LDR is looking for a dependable full-time Program Coordinator (PC) that is an organized self-starter who can lead with compassion.
As a PC you will work in in-home supports and lead a team of direct support professionals (DSPs) to support people with their day-to-day activities in their homes, communities, and in their relationships. The PC makes a positive difference in the lives of others as they partner with their team, stakeholders, and people they support reach their goals.
Under the direction of the Program Manager, this position is responsible for:
- Ensuring and advocating for the proper care and safety of all clients.
- Directly perform hands-on work and attend training on an on-going basis to respond to client needs and role model appropriate behaviors within the agency
- Providing positive, accountable leadership within the organization. Recognize and motivate direct reports, coach and train direct reports for operational excellence.
- Develop and implement plans determined by the 245D rules and regulations
- Participate as a team member for all client meetings, implement and ensure CSP Addendum, Self-Management Assessment, and Individual Abuse Prevention Plans are current, reviewed and updated annually or as needed.
- Provide direct care to clients as necessary (direct care is about 50-75% of the case load time)

Requirements:
· Experience providing direct care services to persons with disabilities or persons aged 65 and older AND/OR post-secondary education that totals 4 years
· Ability to learn, understand and implement 245D licensing requirements
· Strong written and verbal communication skills
· Strong organizational skills and ability to manage multiple priorities and exercise time management
· Critical thinking skills and ability to independently problem solve
· Ability to work collaboratively with a team, leadership experience preferred
· Must be able to lift and transfer recipients, adhering to the physical demands of the job and safety requirements
· Able to travel between locations as necessary: hold a valid driver’s license and have reliable transportation
· Pass a background study completed by DHS
· Computer skills
· Strong passion and desire to develop staff and the individuals we work with to their full potential
This is a full-time position with a mix of office time (Courtland) as well as direct care (in St Peter/Mankato area). May include early evenings and occasional weekends (every other, if necessary). Hours are flexible between 8 am and 8 pm.
