JOB SUMMARY
Evaluates and recommends appropriate medication therapy to attain appropriate and efficient patient outcomes by collaborating directly with oncologists, nurses, and other healthcare providers. Leads the way in the development, supply, and evaluation of overall pharmaceutical care along with quality assurance programs. Serves as a resource to pharmacy colleagues and other hospital colleagues as a drug expert. Assesses patient's health conditions and evaluates medication profile for optimal patient outcomes. Monitors therapy and goals of care while documenting where appropriate. Participates in professional development activities. Participates in drug cost reduction programs.JOB DETAILS AND REQUIREMENTS
*Up to $10,000 Sign on bonus
Type: Full-time, 80 hours per 2 week pay period
Typical hours: Monday-Friday, hours may vary between 7a-5p
Education:
- PharmD from an ACPE-accredited School of Pharmacy or a Doctor of Pharmacy from an ACPE-accredited college of pharmacy- required
- PGY-1 residency or 3 years of equivalent experience- required
- PGY-2 in Oncology- preferred
License/Certifications:
-Active pharmacist license in good standing with the appropriate State Board of Pharmacy,
- Board Certification in Oncology (BCOP)- required within 2 years of hire date
- State Specific Medications Therapy Management Certification- required
Experience:
Minimum of one (1) of the following: Minimum of one (1) year of hospital clinical pharmacy experience or at least 2 years of general hospital pharmacy experience preferred; within area of focus of oncology
