Leading MDs is seeking a Quality Care Coordinator to join the team.
The Quality Metric Coordinator for Leading MDs will be responsible for ensuring timely and accurate reporting of quality metrics for our clinic and long term care ACO programs. This role involves analyzing and interpreting healthcare data, designing and implementing strategies to improve metrics and reporting processes along with the development of process improvement plans to close open care gaps, and collaborating with interdisciplinary teams.
Job Duties
- Chart preparation for quality opportunities and clinical opportunities in advance of patient visit to ensure open care gaps are addressed.
- Collaborate with providers and clinic staff to provide feedback and assist in addressing care gaps or resolving discrepancies.
- Work with scheduling and clinic staff to ensure patients with outstanding care gaps are prioritized for scheduling.
- Collaboration with Accountable Care Organization (ACO) groups and various insurance providers to identify and close gaps in patient care.
- Collaboration with other groups or contracted employees to gather information needed.
- Collaborate with team to compile reports to measure and track quality measure performance metrics.
- Support performance improvement initiatives based off of identification of care gaps.
- Maintain knowledge and stay up to date in quality reporting requirements.
- Strict adherence to all HIPAA guidelines and regulations.
- Management of staff in a healthcare office/clinic setting including day to day operations and workflows.
- Perform other related duties as assigned.
Requirements
Minimum Qualifications
- Certified/Registered Medical Assistant (MA, CMA or RMA)
- Licensed Practical Nurse (LPN)
- Care management experience
- One (2) years’ experience within a healthcare setting
- Understanding of reportable quality metrics
- One (1) years’ experience data and quality abstraction utilizing information from electronic medical records and other forms of information
- Healthcare Effectiveness Data and Information Set (HEDIS) quality reporting experience
- Proficiency in using Microsoft Office programs and web applications
- Ability to work in both independent and collaborative settings
- Demonstrate time management skills and ability to adhere to project productivity time lines
- Strong interpersonal and communication skills
- Strong customer service focus
Preferred Qualifications
- Registered Nurse (RN)
- Office managerial or related experience
- Care management experience
- Other quality metrics abstraction and reporting experience
- EMR experience
- ECW (EClinical Works)- Preferred
