Overview
We are seeking a dedicated Case Manager to join our team in Omaha Nebraska. The ideal candidate will have strong leadership and adaptability skills and experience in behavioral health, child welfare, crisis management, developmental disabilities, and social work. Responsibilities
- Provide case management services to clients in need of assistance
- Conduct assessments to determine clients' needs and develop individualized care plans
- Coordinate with various agencies and service providers to ensure clients receive comprehensive care
- Monitor and evaluate clients' progress towards goals and adjust plans as needed
- Offer crisis intervention and support during challenging situations
- Training employees on programs and individualized support plans/safety plans
- Maintain accurate and up-to-date case notes, documentation, home/environmental/group home visits
Qualifications
- Experience in developmental disabilities, child welfare, mentoring, or related fields is a plus
- Knowledge of DHHS policies, Medicare, and other social service programs is beneficial
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Bachelor's degree in social work or a related field preferred
- Certification in crisis intervention, CPR/First Aid, Medication Aide, or relevant areas is advantageous

Pay: $35,000.00 - $40,000.00 per year
