Key Qualifications
- M.D. or D.O. degree from an accredited medical school.
- Family Medicine/Internal Medicine residency trained from an accredited institution.
- Board certification or board eligible.
- Eligibility for licensure in the state of Connecticut.
- Eligibility for Federal and State Controlled Substance Licenses.
