Youth Services Coordinator
The Independent Living division of The League is seeking a dedicated, hard-working and self-motivated Youth Services Coordinator to join our team. This full-time, non-exempt position is responsible for:
· Providing opportunities for young people with significant disabilities to develop the skills needed to become self-reliant, contributing members of the community through involvement in age appropriate, integrated, community-based activities;
· Increasing access to free and appropriate public education services for young people with disabilities through the provision of Individualized Education Plan advocacy & support services; and
· Assisting young people with disabilities to acquire the goals on the Youth Services Transition Checklist which are guidelines that children and youth need to achieve before entering adult life.
The ideal candidate should have a strong work-ethic, excellent problem-solving skills, and be motivated to serve our consumers.
Essential Duties/Skills/Responsibilities/Qualifications
Service Delivery
· Conduct initial assessment to determine consumer’s individual needs
· Assist consumers in developing initial, intermediate, and long-range goals
· Track and monitor consumer progress
· Provide information and referral services
· Participate in individual and system advocacy activities
· Coordinate community-based activity experiences and support for consumers
· Conduct staff training for community-based partners
· Provide individual peer support
· Organize family networking activities
· Conduct and coordinate consumer/parent/youth professional rights and systems training programs
Public Relations/Marketing
· Conduct public education and awareness activities
· Provide community organizations with training related to disability issues
· Maintain positive relationships with the consumer, community, agencies, government entities, co-workers, and other professionals
Administrative/Clerical
· Manage and monitor the Youth Services program to achieve at minimum the numbers in the outcomes-based evaluation report
· Activate, track, and inactivate consumer files using the League’s policies & procedures.
· Maintain current and accurate consumer files (case notes and data base)
· Track community activities and information and referral contacts
· Gather program outcomes-based evaluation data
Financial
· Determine parent/guardian financial participation for responsibility for community program fees
· Provide Finance Department with billing information related to financial participation in community program fees
Planning
· Work with community programs to develop programs and services that will benefit youth with disabilities
Bachelor’s degree (B.A.) from a four-year college or university; or two years of post-secondary education and one- two years of related experience and/or training; or an equivalent combination of education and experience.
