Requirements
- Minimum BS in Social Work preferred.
- 1 year experience in Social Service work in a LTC setting.
- Manage QIDP's/Case managers in ensuring Care Plans are updated and relayed to Direct Care staff to help residents meet goals.
- Ability to motivate, encourage residents; knowledge of how to adapt life skills to the cognitive and functional ability of each resident and attempt to meet goals.
- Ability to handle multiple priorities.
- Possess written and verbal skills for effective communication.
- Competent in organizational & time management skills.
- Demonstrates good judgment, problem solving and decision-making skills.
- Knowledge of state and federal resident long-term care regulations in an LTC setting.
- Excellent communication, leadership, and mentoring skills
