Position Description: Urban Case Manager

Division: Administration

Reports to: Urban Office Coordinator

Worksite: Twin Cities ICWA Urban Offices

Work Hours: M-F, 8:00am — 4:30pm, and as needed

LLBO Status: Exempt

License/Certification Requirements: Valid MN Drivers License, current insurance or be insurable.

Summary: Responsible for providing comprehensive case management services to children and families who are involved in, or is at risk to be in the child welfare system. The goal is to work directly with children & parents with the intent to prevent or reverse the removal of children from the home. Provides support services to enhance or ensure the family's ability to achieve child safety, to teach and reinforce positive family relationship skills and to promote the well-being and permanency of children in a stable home environment. This person works as part of a team to carry out the following responsibilities.

Education Requirements and Experience:

Bachelor's or AA Degree in Social Work (BSW), Criminal Justice, Psychology or other Human Service related field is required or; Two years of experience in a Social Work, Criminal justice, Psychology or the Human Services field.

Experience working with Native American Indian children and families is preferred. Experience in Microsoft Office applications is preferred.

Additional Qualifications and Skills:

Knowledge of the; Minnesota Indian Family Preservation Act and the Indian Child Welfare Act is required.

Knowledge of Minnesota Statutes as they apply to child maltreatment is preferred.

Good verbal, written communication and customer service skills is required.

Ability to pass pre-employment, random drug test, criminal and Social Services background check. Native American preference.

Duties/Responsibilities:

Knowledge of the Leech Lake Tribal Codes relating to child welfare services, is required or willingness to learn.

Be on call and respond to resolve crisis situations within the community as needed.

Familiarity with the Title IVE Tribal/State Agreement is required or willingness to learn.

Familiarity with the Urban area Human Services programs and surrounding county social service agencies is required or willingness to learn.

Manage a potentially high client case load of families residing within the metro area.

Develop case plan with families and monitor to ensure goals are achieved.

Provide support and motivation to families in reaching their goals.

Work individually with families on parenting issues and building parenting skills.

Collaborate with other programs/resources to facilitate family group activities for the purpose of strengthening family structure and build on family values.

Conduct scheduled and unscheduled home client home visits.

Arrange for necessary meetings with other resource agencies and provide referrals to ensure the family receives appropriate, culturally relevant services.

Coordinate services to work with the family, relatives & children in resolving family crisis situations.

Attend Case Review Team meetings.

Provide court reports to appropriate individuals and attend court hearings involving the family. Facilitate supervised visits to mentor & model positive parenting roles.

Provide face-to-face visits with the family on a minimum of twice per month.

Become knowledgeable with the North Star Care for Children.

Assist clients in completing eligibility benefit applications.

Submit accurate targeted case management billing forms ensuring documents are on file for eligible services.

Maintain client records through State reporting system (SSIS).

Complete service arrangements to authorize payments for support services.

Submit adequate and timely reports to management upon request.

Attend the required trainings listed in the "Required Training" form relative to the job position.

Maintain a respectful working relationship with colleagues, Leech Lake Reservation program employees, county human services staff and other community stakeholders.

Meet deadlines and prioritize workload.

Work effectively with minimal supervision.

Attend program activities outside normal working hours as necessary.

Support and maintain sensitivity to Native American culture, lifestyles and values.

Attend and or conduct all mandatory meetings and trainings as required.

Maintain confidentiality and professional ethics at all times.

Travel in and out of the state as needed.

Adhere to policies governing staff conduct.

All other related duties as assigned.

Physical Requirements:

Ability to; lift 30 pounds or more and use proper body mechanics.

Be able to sit or stand for extended periods of time.

Subject to internal working conditions, non-smoking environment.

Pay: $29.46 per hour
