The Stroke Coordinator provides clinical expertise and leadership within the interdisciplinary team responsible for the development, ongoing evaluation and improvement of the Stroke Program. The primary responsibilities of the Stroke Coordinator are: 1) to work in collaboration with the Stroke Program physician leadership to promote optimal stroke care and to ensure integration throughout the organization, 2) to support improvement of clinical outcomes through the development, implementation and refinement of best practices/processes centered around evidence-based practice for the care of stroke patients, 3) to provide program management overseeing the implementation and evaluation of initiatives to improve quality of care, including disease specific care certification, 4) to serve as a role model, educator and resource to support professional development and raise community awareness to stroke prevention, recognition and treatment.
Responsibilities
GENERAL ACCOUNTABILITIES AND ESSENTIAL FUNCTIONS:
- Effectively assess inpatients using neuroscience nursing; where appropriate, provide consultation, coordination and advocacy related to patient care in area of specialty.
- Empower clinical decision-making by promoting the development and use of critical thinking skills, technical skills, and interpersonal communication skills unique to this patient population.
- Provide timely feedback and coaching on stroke care protocols and site-specific processes to stroke care team members.
- Resource for questions about NIHSS, care protocols and complex stroke-related patient issues.
- Serve as clinical liaison to research team when research studies are conducted.
- Coordinate and participate in multidisciplinary stroke rounds.
- Consult on and support the communication of the daily plan of care with all team members.
- Assess patient care processes and the outcomes of care; plan and intervene as appropriate to promote delivery of optimal stroke care.
- Participate in systemwide forums to standardize care protocols and processes and support system integration of best practices.
- Support a standardized process and content for stroke education; review materials to ensure relevance and accuracy.
- Collaborate with bedside nurses to provide patient and family stroke education.
- Work together with LH Stroke Program, Nurse Managers, Clinical Nurse Specialists, and unit educators to assess needs and provide ongoing education to support staff competency in stroke patient care.
- Partner with other stroke team members to deliver community education around the prevention, recognition and treatment of stroke.
- Work closely with the LH Stroke Program to ensure data is accurately abstracted and reported to support continuous improvement in stroke patient care and for compliance with LH Stroke Program protocols and regulatory requirements (e.g.,Core Measures, Get With the Guidelines)
- Be knowledgeable about hospital Stroke Program performance, goals, and areas of opportunity and support the communication of performance widely throughout the stroke care team and organization.
- Actively work with unit leaders, educators and other care team members in pursuit of performance excellence and improvement.
- Collaborate with the LH Stroke Program team to prepare the organization for disease-specific care certification surveys.
Qualifications
Education:
Academic degree in nursing (BSN or MSN/MN) required. Internal candidates promoting may meet the degree requirement with verification of bachelor’s or master’s degree in progress with an expected completion date within 2-years of the promotion date; progress will be reported periodically to the site Chief Nursing Officer (CNO).
Experience:
2-3 years of expertise in the provision and coordination of care for stroke patients required. Experience leading performance improvement efforts and disease-specific program management strongly preferred.
Skills:
Demonstrated skills in patient assessment, multidisciplinary collaboration, problem solving, data analysis, leading change, teaching, coaching, written and verbal communication. Proficiency in Microsoft Office Suite and clinical information systems.

- Current applicable state RN License required.
- For RNs working in both Oregon and Washington, Oregon and Washington RN licensure required.
- BLS through AHA.
- NIH Stroke Scale Certification.
- SCRN, CNRN or NVRN-BC preferred.
Pay Range
USD $53.61 - USD $80.07 /Hr.Salary : $54 - $80
