Based in Watertown, South Dakota, this role focuses on cultivating relationships with hospital discharge planners, managing referrals, and creating strategic community partnerships to ensure smooth transitions for residents from acute to post-acute care.
Some Key Responsibilities :
- Build and manage relationships with hospital discharge planners, social workers, and other referral sources across your region.
- Strategically market Avantara’s services to hospitals, clinics, and healthcare providers to drive referrals and build long-term partnerships.
- Collaborate with facility leadership across your region to streamline admissions workflows and enhance the resident and family experience.
- Identify opportunities for census growth by analyzing referral patterns and market trends within your region.
- Represent Avantara at healthcare events, community expos, and networking opportunities to elevate brand presence.
- Track and report referral activity, conversion rates, and other key metrics to demonstrate ROI and support strategic growth plans.
