RN Admission Nurse - Home Health (Full-Time / Per-Diem)
Compensation: $150 per visit mileage reimbursement

Your work will involve creating personalized plans and coordinating with a collaborative team to ensure clients receive the right care.
Job Duties:
- Conduct thorough OASIS admission assessments
- Develop individualized care plans for new clients
- Suggest additional services as needed to support holistic care
- Communicate with clients, their families, physicians, and care team members
Ideal Candidate:
- Active RN license in Minnesota
- Reliable vehicle with insurance and a valid driver’s license
- Experience with Medicare and OASIS documentation
- Ability to work independently and manage care with excellent organizational skills
