What are the responsibilities and job description for the Admissions Coordinator position at Legacy Home Health Agency?
We are seeking motivated individuals to coordinate the start of care for new patient/clients for the in-home provider care program (also known as PAS or PHC services) at Legacy Home Health Agency, Inc. by providing a channel through which patients/clients or caregivers can seek solutions to problems, concerns and/or unmet needs.
Responsibilities will include:
- Answering high volume incoming calls regarding patient/client services.
- Processing patient information in agency database.
- Communicating with state caseworkers and insurances to coordinate services.
- Follow up on client status as needed.
- Conduct home visits to initiate services.
- Travel to physicians offices to acquire documentation if needed.
- Perform other duties as assigned.
Job Qualifications:
- Customer service experience required.
- Work experience in PAS/PHC Coordination or in a healthcare related field preferred.
- Bilingual in Spanish required.
