Job Description:
At Legacy Hospice, Inc., we are seeking a highly skilled Patient Care Manager to join our team of dedicated professionals. This role involves conducting comprehensive assessments of patients and families to determine hospice care needs, developing, initiating, and revising care plans based on patient needs and status, administering medications and treatments as prescribed by physicians, and offering counseling and support to patients and families, including education on self-care techniques.
Requirements:
- Graduate of an accredited nursing school with 1-2 years of recent acute care and home care experience, current state licensure, and Active CPR certification.
- Valid driver's license, personal automobile, and insurance coverage.
- One year of hospice care experience is preferred.
- Case Management experience and supervision of hospice aides is a plus.
Estimated Salary:
$85,000 - $95,000 per year, depending on experience and qualifications.
