Job Summary :
Coordinates all resident services for the Memory Support residents. Ensures resident satisfaction, engagement and achievement of quality of life.
Essential Job Functions :
- Provides input on performance review for memory support CNAs and activity care team.
- Coordinates resident admissions to Memory Support, working with families to obtain residents personal history to develop a resident specific service plan.
- Motivates and engages residents to function independently.
- Establishes and maintains a close liaison with residents and families.
- Conduct family educational sessions on dementia and develop a library / resource center providing information on dementia.
- Maintains all records and communications as required by state agencies.
- Implement Teepa Snow programming, utilizing available resources and incorporating person-directed care in coordination with the Healthcare Lifestyles Manager.
- Provide and track on-going dementia care education, and computer-based training to staff of Teepa Snows programming. Become a certified trainer for her program.
- Responsible for the transfer / discharge process whereby residents are transitioned smoothly to higher levels of care, as required. Counsel with resident representatives as needed.
- Conducts ongoing care management meetings. Counsels staff on specific problems encountered by residents. Communicates with supervisor and / or appropriate agencies.
- Other duties as assigned by the Supervisor.

Educational Requirements and Experience :
Knowledge, Skills, and Abilities :
Ability to communicate effectively in writing and verbally, speaking the primary language of the residents.
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.
Proficient in Microsoft Office including Word, Excel, PowerPoint and Outlook.
Must demonstrate a warm, positive, and outgoing personality and behavior
