Case Manager sought for an outpatient level expansion of a Substance Use Disorder treatment center in Green Springs, OH. Chemical Dependency Counselor Assistant (CDCA) certification is REQUIRED to apply. Responsibilities
- Manage patient care by coordinating services and resources to meet the needs of clients.
- Conduct patient assessments to determine appropriate care plans and interventions.
- Utilize medical terminology and knowledge of physiology to communicate effectively with healthcare providers.
- Ensure compliance with HIPAA regulations and maintain confidentiality of patient information.
- Develop and implement discharge plans in collaboration with healthcare team members.
- Maintain accurate and up-to-date medical records for each client.
- Provide intake services to new patients and assist with the admission process.
- Utilize case management skills to advocate for patients and coordinate care across various healthcare settings.
- Collaborate with interdisciplinary teams to address the complex needs of patients, including those with addiction, mental health, and behavioral health issues.
- Utilize experience in hospital and acute care settings to provide comprehensive care to patients in various stages of illness.
- Apply knowledge of pediatric care to support the unique needs of children and their families.
Job Type: Full-time
Pay: $37,000.00 - $47,000.00 per year
