Job Description
A professional working in the field of drug and alcohol providing a full range of case management services. This person will be responsible for ensuring that clients with D&A problems have access to services in a continuum of care by identifying, referring, and seeking authorization for appropriate services. The employee will perform the work in accordance with state/federal laws, as well as established regulations, policies, and procedures; but is expected to exercise initiative and independent judgment while preforming the duties as a case manager.
Candidate will have an understanding of human services obtained through formal training at the college level or he/she will have at least two years’ experience in the field of substance use or other human service.
Case Manager’s duties include:
1. Facilitates client’s entrance to treatment.
2. Work with funders to garnish authorization for funding for appropriate level of drug and alcohol services based on ASAM criteria.
3. Provide bed searches and precertification for clients awaiting treatment within Northampton County Prison.
4. Provide bed searches for Intake Unit inpatient treatment referrals.
a. Monitor client placement progress through close contact with treatment providers.
5. Conduct reports and follow up with inpatient/outpatient referrals as well as document/record outcome.
6. Serve as a client advocate.
a. Assists client in accessing community and available resources to meet client needs.
b. Send clinical documentation to treatment providers on behalf of client.
7. Serve as a central point of contact and coordinator of care with all other service providers, marketers, agencies, community resources, to ensure coordination and continuity of services.
8. Work in close cooperation, build relationships with treatment and supportive agencies and other service resources.
9. Record/document all client contact as well as collateral contacts with treatment providers and entities involved.
10. Attend ongoing training in the field to keep informed of trends and changes in state and federal laws as well as Department policy and procedure.
11. Attend meetings/events in the community to build upon knowledge of available community resources.
Other essential duties:
- Conduct drug and alcohol assessments and determine appropriate level of care based on ASAM criteria.
- Assist with scheduling assessments/verifying insurance coverage/collecting payment.
- Schedule assessments for Northampton County Central Court program.
- Complete follow up.
- Submit DUI court referrals to Central Court weekly.
- Enter required documentation in PA WITS for case management services when applicable.
- Cover front desk when/if administrative assistant is unavailable.
Job Type: Full-time
Pay: $18.00 - $21.00 per hour
Expected hours: No more than 40 per week
