Our mission is to provide a comprehensive continuum of care through recovery-oriented services, including inpatient and outpatient programs, medication-assisted treatment (MAT), and various pathways to recovery.
TTI is thrilled to be opening a new outpatient program in Slatington, PA! We invite you to become a part of exciting initiative to bring quality services to this under served area. Selected candidate will begin work in our Allentown facility for training, before transitioning to Slatington upon opening.
Summary
As a Case Manager at Treatment Trends, Inc., you will play a vital role in guiding individuals through their recovery journey. Located in a supportive environment, this position is essential to our mission of empowering clients by providing personalized care and resources tailored to their needs.
Responsibilities
- Conduct level of care assessments to determine clients' needs and develop individualized service plans.
- Provide ongoing support to clients throughout their recovery process.
- Coordinate with healthcare providers, social services, and community resources to ensure holistic care.
- Complete documentation timely and accurately
- Advocate for clients' rights and access to necessary services within the community.
Qualifications
- Bachelor's degree or higher in an approved field.
- Minimum of 1 year Case Management experience, preferably in an addiction treatment or similar setting.
- Excellent communication skills for effective collaboration with clients and stakeholders.
- Ability to develop individualized service plans based on comprehensive assessments.
- Prior completion of DDAP Case Manager Training and a training on Level of Care Assesement training is a plus.
Recent staffing regulations for counselors expanded acceptable Bachelor’s degrees beyond ‘related’ human services such as counseling, psychology, social work or criminal justice to include anthropology, communications, nutrition, sociology, education, divinity and pastoral ministries.
