JOB DESCRIPTION
Job Title: Case Manager
SUMMARY
The Case Manager of Lehigh Valley Adult Services will be responsible for developing, executing, and managing all day-to-day aspects of the client services by overseeing clinical and administrative operations and managing case files. The Case Manager will facilitate patient care by assessing needs, evaluating treatment options, and coordinating care. They are to act as job coaches and day center support staff, as a leader and supervisor as part of the integral team.. The Case Manager will offer insight of how decisions could impact the program, participants and staff to the leadership team
ESSENTIAL DUTIES AND RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO:
- Completing quarterly and annual assessments of new individuals to determine abilities.
- Participating in the individual plan process, development, team reviews and implementation.
- Providing effective leadership in all areas of the adult day center including client-centered care initiatives, personnel management, and quality and safety of programming
- Completing client satisfaction surveys each month
- Scheduling of all medical appointments and follow-ups and attendingappointments as needed
- Ensuring the nutritional guidelines for all individuals are followed as written
- Completion of intakes and maintaining contacts with referral sources as needed
- Training, developing the performance of day center staff and job coaches
- Auditing and addressing staff documentation for timeliness, quality and accuracy while coordinating with the Community Integration Director
- Evaluating staff performance and completing corrective/disciplinary action if required
- Monitoring client services to ensure effectiveness, quality of care compliance and identifies areas of improvements.
- Participating in the referral process
- Ensuring all staff are trained in the individual’s plan; to include assessments and health promotions/protocols
- Other duties as assigned.
QUALIFICATIONS
- Successful completion of FBI fingerprint (as required), PA Criminal Background Check, Child Abuse Clearance, Drug Screen, and complete a Physical
- Excellent communication skills
- Analytical Skills
- Complete knowledge of the IDD and Department of Aging Regulations
EDUCATION AND/OR EXPERIENCE
- Bachelor’s in human services Field or similar is required (Maters is preferred)
- Minimum two (2) years supervisory experience
- Understand and comply with Incident Management
- First Aid Certification required; Instructor level training preferred
Hours of Work / Compensation
Fulltime, including possible nights and weekends
Language Skills
Ability to read and interpret documents such as procedure manuals and work instructions. Ability to write routine reports and correspondence. Ability to speak and communicate well with customers and staff.
CERTIFICATES, LICENSES, REGISTRATIONS
Must remain current and updated
Must complete 48 hours of training Annually
Certified Investigator preferred, or willing to obtain certification within the first three (3) months of employment.
