Job Summary:
As a CIE Case Manager at Human Resources Center, Inc., you will be pivotal in supporting individuals with disabilities by facilitating access to essential services and resources. This position is vital in helping clients achieve their personal goals while promoting community integration and independence.
Position Requirements:
- Education & Experience: Bachelor’s degree in a related field with two years of relevant experience.
- Licenses & Clearances: Valid PA Driver’s License, PA Criminal Record Check, Child Abuse Clearance, and Federal Background Check.
- Certifications: First Aid, CPR, and CPI certification within an acceptable period.
- Physical Exam: Required by an agency-approved physician.
Essential Duties and Responsibilities:
- ISP Meetings: Participate and assist in developing annual outcomes for consumers.
- Documentation: Develop progress notes, review alerts for ISP revisions, and ensure accurate authorizations.
- Coordination: Facilitate communication with various stakeholders and schedule ISP meetings.
- Job Development: Assist with job placement, training, and ongoing support for consumers.
- Reporting: Generate reports for funding agencies and document program requirements.
- Training: Provide direct training at job sites and coordinate schedules for job coaches.
- Evaluation: Interview and evaluate consumers for employment services and suitable job placements.
- Transportation: Arrange and assist with transportation to work sites.
Additional Responsibilities:
- Client Support: Assess client needs, develop individualized service plans, and provide ongoing support.
- Crisis Management: Utilize crisis management techniques to address urgent client needs effectively.
- Follow-ups: Conduct regular follow-ups to monitor client progress and adjust service plans as needed.
- Motivational Interviewing: Implement strategies to empower clients in their decision-making processes.
- Advocacy: Advocate for clients’ rights and promote their interests within the community.
Qualifications:
- Preferred Education: Bachelor’s degree in social work, psychology, or a related field.
- Experience: Experience in case management or social services, particularly with individuals with disabilities.
- Skills: Strong crisis intervention skills, excellent communication, and ability to work collaboratively within a multidisciplinary team.
- Techniques: Familiarity with motivational interviewing techniques is highly desirable.
- Knowledge: Knowledge of local resources available for individuals with disabilities is a plus.
- Additional Experience: Mentoring or supervisory experience is beneficial but not required.
Physical Demands:
- Frequent standing, sitting, walking, talking, and hearing.
- Occasional lifting/moving up to 50 lbs using proper body mechanics.
