What are the responsibilities and job description for the Director of Community Living position at LEL Home Services?
Director of Community Living
The Director of Community Living (DCL) is responsible for the day-to-day oversight and management of all Individual services for all of LEL Home Services (LEL) 24 hour home locations. The DCL will be responsible for facilitating the execution of new 24 hour home locations. The DCL will support LEL’s mission, vision, values policies.
Reports to: Executive Director of Program Services
Supervises: Home Managers and Direct Support Professionals
Director of Community Living will:
- Provide direct supervision of all Managers and functions as a Home Manager while maintaining his/her own active case load of Individuals.
- Responsible for the day-to-day oversight and management of all twenty-four (24) hour Individual services
- Responsible for facilitating the execution of new 24 hour homes throughout the state
- Works closely with the Executive Director of Program Services to ensure compliance with CARF Standards and all waiver regulations
- Works closely with the Director of Individual Finance and Human Resources to assure that the files of Direct Support Professionals and Individuals are maintained in accordance with applicable laws and LEL’s Policies and Procedures Manual
- Follows up and processes all quality assurance, reportable incidents, complaints, occurrences and change of status reports
- Keeps State Director of Program Services well informed of important issues and problems
- Assumes on-call responsibilities as scheduled
- Takes a proactive approach in addressing Individual/family issues and in support of company operations
- Works closely with Individuals and their families, other service providers, case managers, BDDS managers and community resources to create and maintain a cohesive and effective support team for the success of every Individual
- Works closely with all direct reports and the Individual Support Team (IST) to ensure all key elements of the Individuals’ IST plan are identified and addressed including:
- Risks
- Appropriateness of services
- PCISP goals
- Current medications
- Health and safety issues
- Environmental assessments
- Ensures that all elements are updated as needed
- Ensures all Individuals’ Blue Books and other Individual documents are maintained and available in accordance with all applicable rules, regulations and LEL policies
- Attends quarterly and annual meetings with the IST and other team meetings as required
- Demonstrates competencies as a Direct Support Professional by providing assistance to Individuals as they pursue the Good Life.
- Will support LEL’s mission, vision, values and policies and is an advocate for people with disabilities.
- Any other duties requested by LEL
Qualifications:
- Bachelor’s degree in social work, psychology, or a health care related field
- Meets state criteria as a QIDP
- Has a minimum of three (3) years of experience as a Direct Support Coach
- Experience and skills necessary to perform services listed above
- Has a minimum of three (3) years of supervisory experience of shared, supported living locations:
- On-call responsibilities
- Scheduling of 24/7 staff
- Supporting Individuals to create a positive home setting
- Demonstrates initiative in managing issues and problems
- Strong interpersonal and relationship building skills
- Customer service oriented
- Satisfies all requirements under applicable law
- A positive and pleasant attitude
- Ability to work independently and meet deadlines
- Strong organizational skills and close attention to detail
- Strong problem-solving skills
- Excellent written and oral communication skills
