What are the responsibilities and job description for the Reentry Case Manager position at Leon County Sheriff's Office?
- Perform interviews and assessments to determine offenders' risks and programming needs.
- Ability to work within a correctional environment, specifically with inmates assigned to the Leon County Detention Facility.
- Ability to prepare case history reports and recommend program assignments.
- Ability to review work and document necessary information of all assigned offenders to determine progress or necessity of changing assignments/classification.
- Ability to assist the classification unit for caseload needs.
- Ability to counsel offenders on personal problems, employment resources, vocational skills, educational training needs, family problems and institutional adjustment.
- Ability to facilitate programs to offenders which are designed to assist with successful re-entry back into the community following release from incarceration.
- Ability to develop, monitor, and amend client transition plans in line with re-entry programs and services.
- Ability to establish a collaborative network of internal and external partners to provide support, pre and post release services for offenders, that can involve family members, community resources and social services agencies. Internal partners include, but are not limited to, law enforcement, school resource, CSMB, youth services, and intelligence.
- Communicates daily/weekly/regularly with fellow case managers, Detention Deputies, and Behavioral Health Care Staff (YesCare and Apalachee Center) to streamline and reduce instances of service duplication.
- Ability to identify and recommend programming, social services, referrals, and focused case management on identified high risk offenders.
- Ability to collect, organize, and report statistical data (as required) on a weekly, monthly, quarterly, and annual basis.
- Ability to perform proper research on treatment, transitional, and employment services.
- Ability to facilitate classes and programs.
- Ability to research correctional self-improvement programs that assist with controlling and overcoming problems.
- Ability to understand and carry out oral and written instructions.
- Ability to research and build community partnerships.
- Perform any additional assignments as deemed necessary by supervisors.
- Ability to work closely with others as a team.
- Ability to effectively communicate both orally and in writing.
- Ability to prepare and present clear, accurate, concise, and objective written and oral reports.

Performance Measures
- Maintain and manage an offender caseload, as assigned.
- Schedule, manage, complete, and document, assessments, case plans, and contact meetings as instructed by the Reentry Programs Supervisor.
- Report deliverables to Reentry Programs Supervisor in weekly case staffing meetings.
- Enter data into Apricot as designated per department expectations.
- Bachelor's degree from an accredited college or university required; majors in Social Work, Sociology, Psychology, Criminology, or Social Sciences preferred; AND
- Experience working in the criminal justice field preferred; AND
- Demonstrate skills in Microsoft Office products (i.e. excel, word, publisher. etc) preferred.
- Demonstrate skills with data tracking and report writing.
- Possess and maintain a valid driver's license with a satisfactory driving record.
- Demonstrate successful interpersonal skills, particularly with the offender population.
- Program facilitation experience preferred and highly desirable.
- Knowledge of core Detention practices relating to reentry services highly desired.
- Must be able to lift, push, carry or pull a minimum of 50 pounds independently. Position has an increased risk of occupational exposure to hazardous and infectious materials.
- Duties are performed within a Detention Facility and requires varying degrees of standing, sitting, and walking throughout the workday.
Salary : $47,550 - $76,080
