- Conduct comprehensive assessments of clients' needs and develop individualized care plans
- Provide case management services to clients, including coordinating and monitoring their access to necessary resources and services
- Collaborate with other professionals, such as social workers, counselors, and healthcare providers, to ensure holistic care for clients
- Advocate for clients' rights and assist them in navigating complex systems and processes
- Monitor clients' progress towards their goals and make necessary adjustments to their care plans
- Document all client interactions, services provided, and progress made in case management software or databases
```Experience```
- Bachelor's or Master's degree in social work or a related field
- Previous experience in case management or a similar role preferred
- Knowledge of social work principles and practices, crisis intervention techniques, child welfare systems, behavioral health services, addiction counseling approaches, motivational interviewing techniques, workforce development programs, Section 8 housing assistance, supervising staff members, and Medicare regulations
- Strong communication skills with the ability to effectively interact with diverse populations
- Excellent organizational skills and attention to detail
- Ability to work independently and as part of a team
- Proficiency in using case management software or databases
