Responsibilities
- Evaluate patients' functional status, physical therapy needs, and develop personalized plans of care.
- Collaborate with physicians to ensure effective treatment plans and coordinate with other healthcare team members.
- Report patients' responses to treatment and any changes in condition to nurse supervisors and physicians.
- Treat patients to relieve pain, restore function, and maintain maximum functionality.
- Inform and educate patients, family members, and healthcare team members about treatment plans, progress, and goals.
- Visit patients within the agency's service area and other nearby locations as needed.
- Submit progress notes per LHC policy and complete evaluations within state-specific guidelines.
- Participate in staff conferences, committees, and weekend rotations as required.
Education and Experience
- Licensure Requirements:
- Current Physical Therapy licensure in Louisiana
- Current CPR certification
- Valid driver's license and vehicle insurance, with access to a dependable vehicle
