Job Summary
The Home Health Physical Therapist Assistant performs treatment and provides exercise instruction and patient education to improve patients' well-being under the established plan of care.
Responsibilities
Under the supervision of a Physical Therapist, this role follows all state laws governing physical therapy in home care and performs only qualified procedures. The PTA makes recommendations for changes in the plan of care to the supervising Physical Therapist.
This position instructs patients, family members, caregivers, and other healthcare team members in evidence-based treatment within the scope of the physical therapy assistant and according to state laws.
The PTA observes, records, and reports to the supervising PT, nurse supervisor, and/or physician the patient's response to treatment and changes in the patient's condition. Coordination with other healthcare team members occurs as necessary.
Visits are conducted within the service area of the respective agency and nearby LHC agencies as needed. Progress notes are completed and submitted per LHC policy.
Schedule flexibility is required to ensure patient care needs are met. Weekend rotations and observed holiday availability may be necessary.
Participation in staff conferences and committees is requested by the agency. Adherence to LHC Therapy Standards of Practice occurs in provision of services to all patients.
All other duties are assigned by management.
Education and Experience
Requirements:
- Current licensure or certification to assist in the practice of physical therapy in the state of practice.
- Current CPR certification is required.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
