About the Role
This is a full-time/part-time opportunity to work as a Registered Nurse in home and community-based services. You will be responsible for providing nursing and personal care to patients in their homes, as prescribed by physicians or authorized by state/program regulations. Your expertise will ensure that patients receive comprehensive care, working closely with other healthcare professionals involved in their care.
- Develop and revise individualized plans of care and/or service plans according to federal and/or state program requirements.
- Ensure plan of care frequency and duration meets the needs of the patient and initiate plan of care revisions as needed with physician approval.
- Collaborate with other community providers to ensure seamless care coordination.
