Position: Housing Specialist
Location: Phoenix, AZ
Compensation: $21/hr - $26/hr, depending on experience (DOE)
Sign-On Bonus: $4,000
Field Work: 80% field work, mileage reimbursement will be provided
Job Summary: We are seeking a dedicated and experienced Housing Specialist to join our team. The ideal candidate will have a strong background in housing support and will provide comprehensive assistance to individuals with serious mental illness (SMI) and behavioral health challenges, helping them secure and maintain stable housing.
Key Responsibilities:
- Provide individualized housing support to clients, including assistance with finding, securing, and maintaining housing.
- Conduct assessments to identify clients' housing needs and preferences.
- Develop personalized housing plans and strategies to help clients achieve stable housing.
- Facilitate housing readiness workshops and group sessions to enhance clients' independent living skills.
- Collaborate with landlords, property managers, and housing agencies to create housing opportunities and support clients in their housing placements.
- Respond to crisis situations and provide immediate intervention as needed.
- Maintain accurate and up-to-date client records using Nextgen software.
- Participate in rotational on-call duties to provide 24/7 support to clients.
Qualifications:
- Associate's degree or higher in a related field is strongly preferred.
- Minimum of 3 years of experience working with the SMI/behavioral health population.
- Experience in housing support and services.
- Strong interpersonal and communication skills.
- Ability to work flexible hours, including holidays and weekends.
- Experience with Nextgen software and CPRP certification is a huge plus.
