Job Summary
The Patient Liaison plays a vital role in supporting the Longevity Health I-SNP (Institutional Special Needs Plan) Medicare Advantage plan within our partnered Skilled Nursing Facilities (SNFs). In this role, you will work directly with Skilled Nursing Facilities (SNFs) teams to ensure smooth enrollment into the Longevity Health I-SNP program, maintain compliance, and help implement strategies to improve patient care. Additionally, you will educate Skilled Nursing Facilities staff, patients, and families on the benefits of the I-SNP program and assist in identifying eligible members for enrollment.

Key Responsibilities
- Serve as the main point of contact between Longevity, SNF leadership, and patients/families.
- Develop I-SNP strategies and set enrollment goals for each assigned SNF.
- Monitor and manage the I-SNP program to meet goals and requirements.
- Provide training and education to SNF staff on I-SNP benefits and procedures.
- Assist in identifying new patients eligible for I-SNP and provide initial program information.
- Communicate I-SNP referrals to Longevity in a timely manner.
- Analyze patient and program data to find areas for improvement.
- Address quality and cost concerns in collaboration with the SNFs.
- Ensure SNFs follow Longevity's compliance standards.
- Lead special projects and other tasks as needed.
Education & Experience
- High school diploma required; bachelor’s degree in social services or Case Management preferred.
- At least 3 years of experience in skilled nursing, long-term care, or a related healthcare setting.
- Ability to work Full-time, Monday to Friday, 8am – 5pm EST.
- Strong communication, and computer skills.
