FLSA: Non-exempt

Reports To: Assistant Director of Residential Services

Physical requirements: Normal Office Environment

Hours of Work: 8:30 - 4:30 Monday - Friday 37.5 hours per week with occasional evening hours required.

Pay Range: The pay range is $23.06 to $33.74

Job Summary: The Admissions Coordinator will oversee all tasks related to the admission of people seeking services. This will include, but not be limited to, screening all referrals, facilitating all pre-admission and admission meetings for day and residential programs, and completing all required admission paperwork for the agency and OPWDD. This position serves as the liaison between Liberty ARC and other community agencies, schools and families, and is responsible for obtaining initial eligibility for individuals and aligning them with services.


Job Qualifications:

  • High School Diploma/GED is required, Associates and/or Bachelor's Degree is preferred.
  • Must have a minimum of two years' experience working under OPWDD services.
  • One year of supervisory experience in any field is required.
  • Must possess polished oral and written communication skills.
  • Ability to communicate effectively and serve as a liaison to primary referral sources.
  • Must be able to read, write and speak the English language.
  • Valid NYS Driver's License that meets agency standards and reliable transportation.

Major Responsibilities:

  1. Monitor all functions of the admission and discharge process at the agency.
  2. Ensure compliance with all regulatory documentation as it relates to authorization of services for all people admitted and discharged from the agency.
  3. Track, record and maintain all documentation as outlined in the admissions and discharge checklists/procedures.
  4. Act as agency liaison at the Regional OPWDD meetings and accept appropriate referrals.
  5. Complete initial screening for all packets received from OPWDD.
  6. Coordinate all screenings with the person, family, care coordinator and screening team.
  7. Serve as the primary point of contact with people, families and Care Coordinators as referrals are being assessed and ensure clear communication with all parts.
  8. Assist in the development and implementation of policy and procedure around admissions for FSS, Day, and Residential programs.
  9. Coordinate all emergency respite stays ensuring informational meetings are taking place with the team members.
