Title: Outreach Navigator (City Funds/SNOFO) - 2 full time positions
POSITION 1: Hours/Shift: 35 Hours/Week (Mon-Fri 8:00 AM – 3:30PM with 30 min unpaid lunch )
POSITION 2: Hours/Shift: 35 Hours/Week (Sunday through Thursday 12:30 PM to 8 PM with 30 minute unpaid lunch break)
Supervisor: Coordinator of The Next Step Resource Center
The Outreach Navigator is part of the Next Step Resource Center and is expected to identify and build rapport with homeless individuals and families living on the street or in vehicles (unsheltered). The Outreach Navigator will facilitate access to basic needs, POWER, and other services at the Resource Center. The position will assist clients in breaking the cycle of homelessness by moving from the street to housing, accessing necessary support services, and rapidly obtaining permanent housing. The Navigator is a team member of the Outreach & Engagement Team and works collaboratively with other outreach workers to carry out a strategic outreach plan. Navigators will provide individualized client support throughout this entire journey by helping each client develop a plan to address their barriers, increase their income, and maintain and sustain permanent housing. As part of the plan, the Navigator will identify each area in which clients will need assistance to accomplish the outlined goals and objectives (i.e. scheduling appointments, applying for public benefits, identifying subsidized housing, etc.)
Essential Functions:
- Support the agency’s mission, philosophies, goals, planning objectives, special initiatives and supports their ongoing development and implementation.
· Work collaboratively with Outreach Navigators to enhance street engagement through site-based services.
· Coordinate visits via ZOOM or other teleconferencing with licensed provider to complete disability verification.
· Respond to requests from various stakeholders to engage with people in encampments and other “rough sleeping” situations.
· Carry a Caseload of 15 people who are unsheltered.
· Provide supportive services in a nonjudgmental manner.
· Distribute engagement items like bus passes, hygiene products, gloves, coats, hats, etc. to provide relief to people experiencing unsheltered homelessness.
· Document progress toward desired outcomes – housing, health and income stability. Maintain client related data tracking systems, including case notes and complete HMIS entries.
· Prepare case-related reports including outcomes, successes and challenges.
· Provide information, referrals, linkages, and advocacy to assist clients in accessing services and resources.
· Assist clients with procuring necessary documents and services such as identification cards, birth certificate, social security income, disability income.
· Identify appropriate options for clients such as supportive housing and employment.
· Attend weekly Outreach & Engagement Team, clinical supervision and team meetings.
· Work nontraditional hours thus it is crucial that excellent communication be maintained with staff working weekdays and partner organizations.
· Network with other agencies, coalitions, business, faith-based groups, and local community to bring vital services to the people served.
- Ensure provision of referrals and support during times of changing mental health, health, SUD and other client needs.
- Ensure creative methods are utilized to support client/tenant engagement.
- Collaborate with Operation CLEAN activities to facilitate:
- Housing stability
- Income stability and access to paid employment
- Health stability
- Supportive social networks
- Appropriate orientation to Liberty Community Services program(s).
- Creation of a client centered and empowering environment.
- Development of effective resources/support teams with client/tenant.
- Utilization of available resources to improve their quality of life.
- Review and adhere to Liberty administrative procedures and employee handbook.
- Represent Liberty and assigned programs to the community, agencies, and individuals.
- Attend regular supervision with the Program Coordinator.
- Participate in agency meetings/activities, assigned workgroups activities and other meetings as required.
- Collaborate with IT on maintaining compliance with CaseWorthy/HMIS/DdaP - computerized data collection and electronic case records.
- Other requests may from time to time be made by the Program Coordinator, Program Director or Leadership Team to assist the agency's overall management.
- Transport clients – as necessary to housing, legal, medical and other appointments.
Qualifications:
- Position requires a minimum associate’s degree with a concentration in human services.
- Experience working with persons with MH and/or SUD
- Experience working with the homeless or marginalized population
- Must be proficient with PCs and Microsoft Office Word & Outlook
- Understanding of Housing First philosophy
- Training in harm reduction, motivation interviewing, de-escalation techniques
- Creativity, flexibility and ability to work in a changeable and fast-paced environment.
- Working knowledge of employment, housing and human services in Greater New Haven. Experience with employment, housing and homelessness services strongly preferred.
- Strong organizational, written, computer, and communication knowledge and skills a must.
- Valid CT driver’s license required.
· Bilingual capabilities a plus.
- Three strong work-related references (upon request).
