Liberty Hospice - Care for the Terminally Ill
We provide our patients with state-of-the-art care and pain management services. Our specially trained staff emphasize strength, dignity, and compassion in patient care. As a hospice nurse, you will work with families and patients to deliver high-quality care.
The ideal candidate has two years of clinical nursing experience and at least one year of hospice nursing experience. You must be licensed as a registered nurse in your state and have a nursing education from an approved school of nursing. The role involves coordination of patient care with family members, physicians, and other healthcare professionals. Additionally, you must participate in Interdisciplinary Team meetings and quality reviews as scheduled by the agency.
Responsibilities:
- Care and documentation of patient care that meets reimbursement guidelines
- Coordinate physical care of patients with family, patient, physician, and Hospice team
- Attend and participate in IDT meetings and quality review as scheduled by agency
- Travel based on agency need as well as weekend/evening and holiday call assignments
A working knowledge of nursing theories and concepts is required. Familiarity with the principles and practice of nursing according to nursing practice act, agencies' policies and procedures, and reimbursement guidelines is also necessary.
