Liberty Resources is currently seeking a Family Peer Advocate to work in our Integrated Health Care Clinic in Syracuse, NY. Position Summary : The Family Peer Advocate serves as a vital support resource for families navigating the behavioral health system. This role is designed for individuals with lived experience who can provide guidance, encouragement, and advocacy to families of individuals with mental health and substance use challenges. The Family Peer Advocate will work closely with families, community partners, and clinical staff to promote access to services, enhance family engagement, and improve overall wellness outcomes. Job Responsibilities :
- Advocate for family needs and preferences within the CCBHC and external agencies.
- Assist families in accessing community resources and services.
- Offer immediate support to families in crisis situations.
- Complete necessary documentation including assessment, progress notes, and care plans as applicable and in accordance with program and agency standards.
- Provide support to families at home or another agreed location, using a strength-based approach to identify and work on family or caregiver goals. Follow the Phases of Individual Family Support to determine when families no longer need one-to-one assistance.
- Attend meetings with parents or caregivers as requested, and link or refer families to community services and supports as needed.
- Facilitate training in parenting skills, both in groups and one-on-one, and lead support groups for families as needed.
- Maintain accurate records of family interactions, services, and outcomes, while following confidentiality standards. Educate families about mental health, substance use disorders, and available treatment options.
- Build and sustain trusting relationships with families, clinical staff, and community partners. Manage multiple priorities effectively to ensure quality support for families.
- Attend training sessions and meetings to stay informed about best practices and community resources. Actively participate in an interdisciplinary team and contribute to comprehensive care plans.
- Participate in individual and group supervision, staff meetings, and professional development, and maintain any required program certifications.
- Develop a process to consider differing beliefs and preferences and incorporate this diversity into care planning and delivery. Acknowledge personal biases and ensure they don’t interfere with care.
- Provide links to services and assist with system navigation. Report incidents as required by Liberty Resources and oversight agencies, and follow agency policies and procedures.
- Use a personal vehicle for travel to appointments when applicable, and handle emergency situations that may require flexibility in time and location.
- Perform other duties, responsibilities, and related tasks as assigned Qualifications :
- Bachelor’s Degree Preferred, High School Diploma Required
- Understanding of the behavioral health system, resources available for families, and the challenges faced by individuals and families in this context.
- Lived experience as a family member of an individual with mental health or substance use challenges, demonstrating resilience and recovery.
- New York State Credentialed Youth Peer Advocate or willing to become credentialed within one year.
- Must have valid NYS driver's license and access to reliable transportation. Salary Range : $50,000 to $55,000.
