Job Summary
Life Dome is seeking a dynamic and motivated Care Coordinator to oversee a team of dedicated Direct Support Professionals. In this role, you will provide guidance, support, and supervision to ensure the delivery of high-quality services. You will also be responsible for coordinating program activities, including administrative requirements, to ensure efficient and effective operations.
Under the leadership of the Director of Programs, the Program Coordinator acts as the direct service team leader. The Program Coordinator facilitates Person-Centered Planning and coordinates individualized services. Supervises the community habilitation and respite staff (DSP), ensuring effective service delivery. Promotes independence and community inclusion of individuals served through creative planning, effective use of systems management, and professional development.
Responsibilities
- Supervise DSP staff to ensure that all operations are occurring as per LDAS policies and procedures and OPWDD regulations/ State and Federal Laws.
- Ensure that all service delivery, habilitative plans, daily and monthly service documentation, individual records, and billing documentation systems are maintained in compliance with all OPWDD regulations, State and Federal mandates.
- Ensure that all DSP staff members receive the appropriate training in accordance with OPWDD regulations as well as promote their professional growth and development
- In conjunction with the Care Coordination Agencies, individual, and/or parent, attend IDT meetings and Life plan reviews for all individuals served if necessary
- In collaboration with the DSP, develop an individualized habilitation plan using a Person-Centered Approach
- Oversight of Sandata EVV systems to support staff work schedules, service tracking, and documentation
- Oversight of DSP recruitment and onboarding procedures, including training records, documentation, payroll submissions, and professional staff development.
- Monitor and track service units to ensure they are being maximized
- Attend conferences, in-services, professional forums, and informational meetings as assigned by the supervisor to enhance and ensure up-to-date knowledge of current mandates as well as continued professional development
- Engage in capacity-building initiatives to expand service reach, including establishing partnerships with community organizations and facilitating community program presentations
- Accountable for implementing supplemental support services including, support groups and one-on-one caregiver coaching
- The Program Coordinator will also act as LDAS marketing and communications lead to promote all LDAS services and improve organizational awareness needed to expand organizational reach.

Job Type: Part-time
Pay: $20.00 - $23.00 per hour
