Join us while we have a $1000 Sign On Bonus ($500 at 3 months and $500 at 6 months. Must average 20 hours weekly or more*).
Our Case Managers in the Community are part of our Individual Supports Program at LIFE and are based in the mid-upper Cape Cod Community at large, this position is not remote.
Applicant must have a high level of PATIENCE, KINDNESS and DEPENDABILITY and possess the ability to support these wonderful individuals in their activities of daily living to help them live productive and happy lives as independently as possible!!!
Case Managers assist individuals we support at LIFE in fulfilling their independent potential by helping them in managing their daily life skills:
- Activities in the community
- Household management and ADL's
- Menu planning & shopping
- Meal preparation
- Personal presentation and hygiene
- Appropriate behavioral and social skills
- Routine fitness
- Transportation needs
- Medical appointment coordination
- Money management
- Access to social and recreational activities, etc.
The hours needed are:
- Hours are flexible but should fall in the range of 10:00am-7:00 pm Monday-Friday
- If you are a match for the position, we will work with you to create a schedule that works for you--schedule flexibility is one of the positives of this position. If Part-Time is what you need--we will do our best to make that work too.
- GREAT BENEFITS for staff working 30 hours on a weekly basis.

Your skills:
- Must have good organizational and strong computer skills.
- Must have excellent verbal and written communication skills.
- Must have the ability to work with the entire team to the benefit of our clients.
- Be creative in your approach to cater to each individuals specific needs.
- GREAT interpersonal skills!
- Flexibility in traveling around the community during working hours to provide top quality assistance to our program individuals.
- A patient and positive attitude and approach!
- Ability to be a good role model.
Responsibilities include:
- Assist, organize, and facilitate activities as assigned or designed.
- Assist individuals with meeting individual goals and objectives in apartment maintenance, personal health maintenance, nutrition, fitness and social skills development.
- Participate in team-related services; staff meetings, case reviews with staff and/or family, etc.
- Complete in the appropriate computer program routine documentation after every session or significant communication with individuals.
- Maintain all required trainings through our Learning Management System and/or in-person.
- Make recommendations to meet the needs that are beyond the scope of the LIFE staff (i.e., medication adjustments, therapy by licensed clinician, private tutor, house cleaner, etc.).
Applicant must project a positive and patient personality and provide ongoing support, interaction, and encouragement to our clients. Our staff are always role models to our clients. Applicant must be able to work independently and as part of the team.
REQUIREMENTS:
- Must be 18 years old
- Experience with Learning Disabled population a plus, but WE WILL TRAIN THE RIGHT PERSON (personal experience or Special Olympics experience is valued)
- Must be able to pass a CORI and fingerprint background, DPPC and have an acceptable driving record per the Department of Developmental Services
- Reliable transportation
