Direct Support Professional (Meaningful Day-WALDORF)
L.I.F.E. Inc is seeking a caring, patient and energetic Direct Support Professional (DSP) to provide care for some of our individuals. The DSP will assist individuals who have varying abilities to become more integrated in their communities by maximizing the person's strengths, interests, and abilities. The aim is for the person to direct their own activities that create a meaningful life including contributing to the community and developing relationships.
Essential Functions :
- Provide direct support, interaction, and supervision of the supported persons, inclusive of all daily needs (meals, personal care, transportation, leisure activities, and community integration)
- Prepares and maintains a detailed weekly schedule of activities for each day of the week to achieve greater independence using person-centered planning and self-determination.
- Assist participants with job readiness skills and provide transportation and on the job coaching as needed.
- Plans and supervises work of individual's, individually or in small groups, in the community or on site.
- Complete and maintain all state and agency required documentation accurately in accordance with stated guidelines and deadlines.
- Develop a vocational profile for each individual showing current strengths and weaknesses as related to general job skills and work habits.
- Maintains positive, professional working relationships through effective communication with individuals, staff, families, and community coordinators for the purpose of collaboration and planning about progress towards personal goals.
- Performs related duties as assigned.
