Summary
Position Summary: The goal of the Discharge Support Specialist is to empower patients in the post-acute selection process as well as create a seamless transition of care in alignment with the core values of LifeBridge Health.
*This is a Part-Time Weekender position that will be scheduled for 10-hour shifts Every Saturday and Sunday.*
Responsibilities:
The Discharge Support Specialist is responsible for educating patients with a recommendation for a skilled nursing or rehabilitation on their post-acute options. This teaching will include a discussion of quality analytics for skilled nursing facilities. The Discharge Support Specialist must possess critical thinking skills and the ability to anticipate barriers to discharge. The Discharge Support Specialist must be comfortable collaborating with an interdisciplinary team. There will be on[1]going communication with patients and families, post-acute care facilities, and the interdisciplinary team. The Discharge Specialist must be able to multi-task, manage numerous priorities, and remain productivity while managing interruptions. Tasks Review all patients with recommendation for acute care discharge to destinations other than their home setting. Specific focus on patients with a discharge plan to acute or sub-acute level of care.
Requirements:
- Formal working knowledge; equivalent to an Associate's degree (2 years college); requires knowledge of a specialized field
- Less than 1 year experience required
- Maryland Board of Nurse issued LPN license
- American Heart Association BLS
