SUMMARY
The Intake & Discharge Specialist is primarily responsible for engaging with Eastside Shelter clients who have been referred through the Baltimore County DSS Coordinated Entry system. The Specialist is responsible for completing intakes and discharges in HMIS and Salesforce in a timely manner. This includes completing the necessary paperwork and obtaining any supporting documentation from the clients as needed. The Intake and Discharge Specialist is responsible for orienting new clients to the shelter program, providing the clients with new bed linen and hygiene items and assigning the clients to a bed space.
PRIMARY DUTIES
- Engage clients to understand services provided by the Eastside Shelter program and the process of obtaining services through the Baltimore County Coordinated Entry System.
- Complete initial client Intake and Assessment using HMIS Wellsky and Salesforce.
- Assist clients with obtaining any missing intake documentation, i.e. picture ID, birth certificates, social security cards, insurance cards, etc.
- Work closely with the Program Director and Clinical Supervisor in assigning clients to Case Managers.
- Update intake forms and other documents used to provide direct client assistance.
- Complete HMIS Exit Assessments, and Salesforce exit forms for discharged clients.
- Provide information to internal and external customers seeking information regarding bed availability.
- Attend monthly all staff meetings and other department meetings as assigned.
- Utilize HMIS for data collection and comply with HMIS and community standards for data entry. This may require entry into more than one HMIS.
SECONDARY DUTIES
- Participate in active networking with other homeless service providers, as well as other community resources.
- Attend monthly staff meetings and other department meetings as assigned.
- Supports a welcoming and positive environment for volunteers and partner agencies.
- Upholds the vision, mission and values of St. Vincent de Paul.
- Assists with and attends special events held by the Center and St. Vincent de Paul.
- Other duties as assigned within the scope of the position.
Qualifications
QUALIFICATIONS
- Bachelor’s Degree & 2 years’ experience working with persons who are materially poor and/or homeless in an urban setting, required. Years of experience will be considered in lieu of a degree.
- Ability to relate in a courteous, comfortable and non-judgmental manner with persons from all ethnic, socioeconomic and religious backgrounds.
- Understanding of and respect for confidentiality.
- Strong computer skills
- Strong organizational and time management skills.
- Good communication and interpersonal skills.
- Desire and ability to work as a member of a team.
- Possess strong knowledge of homelessness, severe and persistent mental illness, and substance abuse.
- Provide positive customer service to all including difficult and challenging populations
- Must be sensitive to and respect cultural diversity amongst clients, staff, and volunteers and able to work with diverse racial, ethnic, and economic groups
