JOB SUMMARY:
The Medical Respite Care Coordinator/Navigator is an essential member of the integrated health care team that supports medical respite program goals of improving pathways to health care for at-risk residents of Maryland.
- Function as a system navigator and care coordinator for medical respite patients, including primary and specialty health care, dental/oral health, behavioral health, diagnostic services, and social services.
- Participate in triage and intake processes for medical respite patients.
- Provide intensive case management and advocacy for at-risk patients including resource navigation, application assistance, transportation, social supports, and housing stability.
- Partner and maintain relationships with other providers and services within the community to provide person centered care.
- Maintain working knowledge of State of Maryland and Federal assistance programs including public insurances, financial assistance gap programs, Social Security Administration, food stamps, energy assistance, etc.
- Aid the patient with private and public assistance program applications.
- Participate in community workgroups and continuing education opportunities for patient advocacy, health care advancements, and entitlement programs.
- Participate in needs assessments to identify barriers to obtaining health care services, available resources, and ultimately diversion of over utilization of emergency department services.
- Provide clinical and procedural support training to other members of the health care team as related to care coordination and navigation processes.
- Participate in the system development and reporting of quality assurance/performance improvement measures and activities.
- Coordinate referrals, specialty visits, and diagnostic services with proper documentation and patient education
- Track care coordination activities via patient registries and specialized databases.
- Provide monthly reports of statistics including care coordination, referrals, individuals served, and relevant outcomes.
- Provide mentorship and training of integrated staff, partnering entities, and professional students.
- Participate in new initiatives, trainings, and development for improved patient care services, targeting at-risk and vulnerable populations.
- Serve as a resource of social determinants of health issues for the integrated medical respite care team and community.
- Participate in community meetings and workgroups relevant to the job description and population served, such as transportation, shelter services, and multi-disciplinary clinical teams.
- Maintain compliance with HIPAA and Confidentiality standards
- Ensure timely and accurate documentation of services provided within the Electronic Health Record and other documentation tools.
- Assist in other duties and projects as assigned
REQUIREMENTS:
- Licensed nurse in State of Maryland, as RN or LPN, with Human Services field experience preferred
- Bachelors Degree from an accredited college or university preferred, not required, or equivalent related experience
- Current BLS Certification from the American Heart Association.
- Strong organizational skills and attention to detail
- Strong interpersonal communication and relationship building skills
- Superior written and verbal communication skills
- Ability to work with various cultural and diverse individuals
- Bilingual English/Spanish preferred
- Understanding of policy and regulations concerning Maryland public assistance programs, specifically Medicaid, Primary Adult Care Program, Medicare, and the Social Security Administration
