LifeConnections is a non-profit human service agency, serving adults with developmental disabilities from Florida to Maine. We are currently accepting candidate applications for the Shared Living Program Manager position, which will be based out of our Dighton, MA office.
Position Title: Shared Living Program Manager
Office Location: Dighton, MA Office
Salary: $52K-$55K/Year Plus Excellent Benefits!
Supervises: Shared Living Direct Support Professionals
Education: Bachelor's Degree Required
Requirements:
- Experience working with individuals with autism, intellectual and developmental disabilities.
- Strong communication, behavior management and management skills.
- Reliable transportation and is required to meet safe driver guidelines as established by employer’s insurance carrier.
- Position requires an extensive criminal history background check.
Summary: Responsible for the coordination of services and supports for individuals with Intellectual and Developmental Disabilities and/or Acquired Brain Disorders.
Duties/Responsibilities:
· Facilitate and coordinate intakes for new referrals assigned to caseload.
· Assist in the certification and/or home assessment process for new Shared Living Providers as assigned to caseload.
· Recruit, train and supervise Direct support Professionals for individuals on caseload.
· Serve as liaison to internal and external stakeholders such as DDS Regional offices and other community agencies.
· Conduct home visits on a weekly basis for individuals in a new home and once per month thereafter.
· Complete monthly home visit forms and ensure all compliance standards are met. Follow up should be conducted in a timely manner.
· Assure coordination of annual physicals, dental visits and other routine medical visits.
· Coordinate regular multi-disciplinary team meetings with the Clinical Intervention Support Team, Behavior Specialist and Nurse Trainers.
· Attend Individual Service Plan meeting and other team meetings as scheduled.
· Facilitate staff meetings on a monthly basis.
· Approve timesheets on a bi-weekly basis.
· Responsible for managing all programs assigned within the allotted budgets.
· In conjunction with QI team and Director, provide oversight to program to ensure compliance with all state and federal regulations.
· Develop and implement daily schedules for community integration including vocational, educational and recreational activities as outlined in the Individual’s Service Agreement.
· Collaborate and effectively communicate with all team members to ensure quality services is delivered to the individuals supported.
· Provide oversight to staff and providers to ensure funding source’s mandatory required documents are completed in an accurate and in a timely manner.
· Facilitate regular monthly supervision, training and teaching sessions with all staff. Facilitate annual reviews and assist staff in professional growth and development. Provide mentoring and leadership to all staff and provide positive corrective feedback and/or training and support as needed, utilizing different training modalities.
· Coordinate and attend all State certification reviews.
· Provide on-call support on a rotation with the Shared-Living team.
Secondary Responsibilities:
· Ensure that personal, medical and nursing care, behavior plans, and other services that are delivered by staff are appropriate and consistent.
· Empower individuals to self-advocate, initiate ideas, exercise choice about needed supports. Provide normal developmental experiences by informing and encouraging individuals to participate in social and cultural activities in the community.
· Perform other job-related projects and activities as requested or directed.
