Job Purpose
The Community Support Specialist will provide and coordinate services that are an integral part of helping Lifecore Health Group’s consumers’ access needed medical, social, educational, and other services in order to attain their highest level of independent functioning.
Primary Duties and Responsibilities
· Provide conscientious mental health care services to adults with severe mental illness
· Participate in all training required by the Department of Mental Health and Lifecore Health Group so as to maintain current credentials
· Adhere to quality assurance guidelines
· Provide all required documentation pertinent to provision of services
· Other duties as assigned
Qualifications
· Bachelor’s degree in Psychology, Social Work or other Mental Health Related field
· Obtain and maintain DMH certification for level of position
· Valid Driver’s License
· Valid Car Insurance
· Reliable Transportation
Job Type: Full-time
Pay: From $29,000.00 per year
