Essential Duties and Responsibilities :
- With client, develop and implement employment support plans.
- Provide assessment, job development, on-site support and coaching as needed.
- Seek out employment opportunities matching customer interests.
- With Supported Employment Director, maintain employment records for individual customers.
- Build capacity within service networks to provide employment supports.
- Facilitate provision of supported employment services within team structure.
- Participate in conferences and interdisciplinary team meetings as necessary.
- Provide and maintain clear and accurate records and documentation.
- Ensure all customer information is accurate and up-to-date in electronic customer record.
- Understand the importance of social role valorization principles in all LIFEDesigns' practices
- Develop natural supports and facilitate community connections
- Participate in one or more community organizations and support customers to do likewise.
- Network with coworkers, the community and other professionals to find employment opportunities for our customers.
- Complete and submit appropriate billing for all employment services.
- Monitor and continuously improve service quality.
- Seek new ways to provide employment support to employers and customers.
- Maintain a minimum billable time determined annually by the Director of Services.
- Set targets for personal improvement based on a regular assessment of performance from supervisor, customers, and all staff.
- Seek opportunities for personal education and skill development.
Qualifications :
Bachelor's degree in related field preferred with 2 years-experience in the field is preferred. A valid driver's license, proof of automobile insurance and acceptable motor vehicle report is required. In this position, reliable transportation is also required. Each employee must attend and participate in staff meetings and training sessions regularly and maintain all required internal and external requirements and certifications.
Working Conditions :
This is a full-time position. Days and hours of work are Monday through Friday, 8 : 30 a.m. to 5 p.m. Occasional evening and weekend work may be required as job duties demand. Occasional travel is required of this position, most during the workday.
