Care Coordinator (RN) – Travel Assignment
Lawrenceville, GA
13-Week Contract | Monday – Friday | 8:00 AM – 4:30 PM
40 Hours/Week Guaranteed
Responsibilities:
- Coordinate patient care plans, ensuring seamless transitions and continuity of care.
- Conduct case management and utilization reviews to optimize patient outcomes and resource use.
- Serve as a liaison among patients, families, medical staff, and insurance providers.
- Educate patients and families on discharge plans, available services, and follow-up care.
- Document care coordination activities accurately and in a timely manner.
- Ensure compliance with hospital policies and regulatory standards.
Requirements:
Required:
- Active Georgia RN license.
- Minimum 2 years of experience in case management, utilization review, or social services.
- Typing proficiency (40-45 wpm) for accurate documentation.
Preferred:
- BSN degree.
- 3 years of experience in case management or social services.
- Certified Case Manager (CCM) certification.
Job Type: Contract
Pay: $31.18 - $52.76 per hour
Expected hours: 40 per week

- 8 hour shift
- Day shift
- Monday to Friday
