Job Title: Director of Case Management
Location: Lima, OH
Contract Duration: 13 weeks with possible extensions
Shift Type: Days (Monday - Friday, 8:00 AM - 5:00 PM, weekends and on-call as needed)
Responsibilities:
- Lead and manage the Case Management department, ensuring compliance with hospital policies and regulatory standards.
- Develop, implement, and monitor case management programs to improve patient outcomes and control costs.
- Collaborate with interdisciplinary teams to optimize discharge planning, care transitions, and resource utilization.
- Provide training, mentorship, and performance evaluations for the case management team.
- Monitor department metrics and prepare reports for senior leadership.
- Address challenges in patient care coordination and ensure timely and effective solutions.
- Maintain active communication with patients, families, and healthcare providers regarding care plans.
Requirements:
- Minimum of 5 years in case management, discharge planning, or cost/quality management programs.
- At least 3 years of clinical experience in a hospital setting.
- Proven supervisory/management experience.
- Active OH RN License (must be current and valid).
- Basic Life Support (BLS) certification.
Job Type: Contract
Pay: $3,850.00 - $4,056.00 per week
