Responsible for overseeing and managing the medical and health care of program participants including providing direct nursing care.Schedule : Part - timeSaturday and Sunday (alternating) 7 am - 3pm shifts (8 hour per shift, 16 hours per weekend)Essential Job Functions
- Monitors the health of program participants and provides professional technical assistance regarding nursing care and medical treatment as needed.
- Responsible for management and documentation as it relates to medical information, referrals, reports, medication administration and data collection.
- Responsible for the completion of medical record audits and review of medical tracking systems.
- Provides oversight and assistance with the self-medication assessment process.
- Provides oversight of MAP and for the administration of medical treatments.
- Orients and trains staff in understanding participants' medical needs and in following prescribed programs and plans.
- Provides leadership and support to team during medical crisis.
- Ensures professional communication with families, DSS, clinicians, and others for assigned participants.
- Provides guidance to staff regarding medical emergencies and informs supervisor of all emergency treatments or 911 calls made.
- Communicates with medical providers regarding major issues.
- Completes nursing notes and documentation for encounters with individuals.
- Processes all human rights issues, incident and accident reports in a timely manner according to agency policy / procedure. Ensures the human rights of program participants are safeguarded in all interventions.
- Responsible for knowing and following agency, departmental, program and site policies and procedures as well as the bargaining unit personnel policies handbook and the collective bargaining agreement.Experience & Education, Certificates
- Licensed Practical Nurse registered with the MA Board of Registration in Nursing Required.
- At least 1 year of experience working with the elderly or persons with developmental disabilities.
- Strong interpersonal, written, and verbal communication skills required.
- Valid driver's license and reliable transportation.
- REQUIRED Current Adult CPR / first Aid certification
- Maintain a clean record and not have any pending disciplinary actions with the Commonwealth of Massachusetts or job status will be reviewed with the outcome to be decided by management.This job description is not all inclusive and does not state or imply that these are the only duties to be performed by the incumbent.
