We are seeking a highly skilled and motivated individual to join our team at Lifes Good Stay Here as a Qualified Professional. As a Qualified Professional, you will play a crucial role in providing exceptional services to our clients and ensuring their overall well-being and satisfaction.
Responsibilities:
1. Assess client needs: Conduct thorough assessments to determine clients' needs, goals, and preferences. Develop personalized care plans based on these assessments.
2. Coordinate care: Collaborate with interdisciplinary teams to develop and implement comprehensive care plans for clients. Ensure effective coordination of services and resources to meet clients' needs.
3. Provide direct support: Deliver high-quality care and support to clients, including assistance with daily living activities, medication management, and emotional support.
4. Monitor progress: Regularly evaluate clients' progress toward their goals and adjust care plans as necessary. Document and maintain accurate records of client interactions and progress.
5. Advocate for clients: Serve as an advocate for clients, ensuring their rights and preferences are respected. Collaborate with external service providers to ensure clients receive appropriate care and support.
6. Educate clients and families: Provide education and guidance to clients and their families on available resources, treatment options, and self-care techniques to enhance their overall well-being.
7. Stay updated: Stay informed about industry trends, best practices, and regulatory requirements related to client care. Continuously enhance your knowledge and skills through professional development opportunities.

Requirements:
1. Education: Bachelor's degree in a relevant field (e.g., social work, psychology, nursing). Master's degree preferred.

3. Experience: Minimum of 2 years of experience in a similar role, preferably in a healthcare or social services setting.
