Title: Support Broker/Case Manager

Hours Status: Full-Time, M – F, 8a-5p, flexible schedule
Pay Range: $17.55 Hourly
Brief Job Description:
Provide case management services for individuals with intellectual and developmental disabilities in Warren and surrounding counties. Primary responsibilities include making home visits, completing assessments, creating/updating plans, and monitoring outcomes.
Travel reimbursement and opportunities to make additional income. Flexible work schedule Monday through Friday, which could include working from home.
Qualifications include:
- Bachelor’s Degree in the Human Services field
- Valid driver’s license and current automobile insurance
