LifeSpring Health System is seeking Care Managers for our office in Jasper, IN. Duties include teaching activities of daily living skills and linking clients with community resources. The Care Manager will provide services to individuals ages 14-30 with a severe mental illness through individual and group modalities. A Bachelor’s degree in social work, psychology or related field is highly desired. Direct experience will be considered in lieu of education. We are looking for caring, compassionate and energetic people who enjoy helping others. Candidates must have excellent personal and professional boundaries and be committed to person-centered, trauma-informed care. Computer skills are required; experience working with Electronic Medical Records desired.
