Description
ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:
- Provide individual, family, and group counseling and education and support to clients and families, skills training, individualized case management, and outreach to client homes
- Provide brief and proactive safety and crisis planning interventions to promote family stabilization and transition to community-based supports and services
- Under general supervision, screen clients for program eligibility, assess client needs and functioning level, and conduct comprehensive evaluations resulting in a DSM V diagnosis
- Develop treatment plans for clients which includes delivery of appropriate services as defined in the detailed treatment plan
- Serve as contact/resource of information for intensive treatment programming; respond to inquiries regarding these services from the public; present information about intensive services at community or agency outreach events
- Attend intensive treatment Learning Collaborative meetings and other associated meetings or trainings as assigned by Supervisor
- Prepare and submit pre-authorization requests, pre/post outcome data, and provide supervisor with regular status report of youth enrolled in service
- Assist with program planning and development activities
- Make appropriate referrals
Qualifications
Required
- Graduate degree in psychology, social work, or other behavioral science
- must hold appropriate CADC licensure or receive the credential/license within 36-months of application
- Knowledge of regulations and laws governing mental health, behavioral, and addiction services, ethics, confidentiality, privacy, and other related areas
- Must be flexible in providing services in variety of settings to best meet needs of clients (office, telehealth, home, community)
- Must be able to communicate effectively in both oral and written forms
- Knowledge of evidence-based practices, including group counseling skills, one-on-one counseling skills, treatment planning, assessments, and referrals
- Experience with electronic medical records and telemedicine highly desirable
- Knowledge of cultural and family issues that can contribute to Lifeways’ development of clinically relevant and effective care.
- Ability to operate computers, copy machine, faxes, and scanner
- Valid driver’s license
- Must pass a criminal background check
Preferred
- In 2nd year of a master's program in a behavioral health field
- Candidate with combination of education and experience consistent with certification as a Qualified Mental Health Professional (QMHP) in the state of Oregon (OAR 291-124-1030)
- Bilingual (English/Spanish) fluency
